Gravestone Token: One-Sided.

Greetings All!
I wanted to make a summoning stone that worked as a kind of phylactery for a tamed undead creature. Would love to hear your thoughts on the premise, I would recommend pairing with the Band of the Bead to increase your players chances of making friends.

Description:
This gem trinket holds a number of stories tied to its origins, ranging from a lover’s last gift, a token of a lost soul, to a lich’s failed phylactery. Numerous tales have been told, and no doubt more will be shared in the future. This unique token has been carved from a solid emerald gravestone. It is adorned with a poured brass skull. This morbid decoration hints at its purpose as a resting place of the dead.
Tabs:
- The user is able to contain the essence of an Undead Creature within this Trinket. As an Action, they can Teleport a selected creature within 30ft into the Trinket.
- The creature must be no Larger than a Medium Creature, and it must be considered Friendly to be placed within the Trinket.
- The Undead Creature’s Challenge Rating must not be higher than the User’s Proficiency Bonus.
- This Trinket can only contain one Creature at a time. Until another creature is contained in this way, the current creature is considered Tethered.
- As an Action, the user can call forth the contained creature into an unoccupied space within 15ft of the user.
- Tethered: If the contained creature is Slain after being called forth, it will slowly regather its essence within the gem with the user’s soul as a tether. This period will take up to 1d4 Days, after which it can be summoned again. If the user breaks attunement during this time, the creature is lost to the void.
