Love this episode - SPN Season 13 got its mojo back after the pitiful Season 12. I really do like that they're taking their time with emotions this season, letting them play out. It's easy to assume that Dean's deep depression was lifted when Cas returned and, to an extent, it was. But here we can see very clearly that he's still far from recovered. Dean would never normally pull a gun on an innocent girl just because he wanted something from her. He's still struggling with PTSD.
