Love the narrative structure of this and how the different threads all tie together. It's just so good. As for Eva, it was revealed last time she was in focus that she had followed Tenma that rainy night from the hospital and she saw Johan coming down the stairs after killing Tenma's patient.

She knows Johan exists and what he looks like because she saw him leaving that construction site where he killed jonkers infront of Tenma. Jonkers was the patient Dr. Tenma had that he bought a cuckoo clock for and was bringing it to him when he ran out of the hospital at night and ran into Johan at that construction site or parking structure or whatever the fuck it was.
