I've compiled a list of the answers for the questions you asked and details I think should be clarified. project arclight seems to have failed due to the destruction of the time machine, our suzuha + mayuri may or may not be dead, but they are at the very least missing (i want to emphasize that the suzuha that we see in this episode is NOT the suzuha we saw enter the time machine in 2011) the time leap at the end of the last episode was not successful and we as viewers were only led to believe that it was for the sake of this twist the suzuha in this episode is not the same suzuha we knew in 2011, this suzuha has been born and raised after the timeleap failed and has never been to the past I don't know if they ever state specifically why the time leap failed, i just chalk it up to too many factors changed with this attempt and not enough care was able to be put into the creation of the device The fact that okabe is still alive is a very important detail in the mind of 2011 okabe, as it shows that even convergence can have loopholes. showing us that just because someone is "dead" does not have to mean that they are dead, only that they are effectively dead in the grand scheme of the worldline. similar to how kurisu didn't actually have to die at the end of steins gate, just be perceived to be dead by that okabe so that his actions would be based upon that fact. now okabe at the end of the episode plans to use the time leap machine to get back to 2011

We do know for sure that Operation Arclight did not succeed, since if it had, Okabe's reading steiner would have activated when they went back. This is the same as in the original when Suzuha went back alone and nothing changed. Their actual fate is unknown.
