I think the whole 69 is supposed to be a play on the fact that the name of the album is "positions". The way I've interpreted the whole thing is really longwinded hahaha that this is a play on her switching positions and that this is her showing the sexual side of her. People have said that it's weird for her to talk about sexual things for a long time but in reality, she's a grown woman and there are multiple sides to her. Just because people have this image of her as innocent from before (which is much less true now) doesn't mean that there isn't a sexual side to her. I also think it might be a play on how she earlier on in her career had to imply sexual things in a rather convoluted way. Like, she makes it convoluted but in reality she's literally spelling it out for us. That's, just how I thought about it.
