She probably does love him, but he is not seeing it. Xander's anxiety-self cares about Anya, he always cares about her, but he remembered her so late, because all his fears and anxieties were overwhelming. Xander's confident self was able to listen to Anya and make her feel better because when she started stressing about having kids and other stuff, sans anxiety he didn't assume it's about him somehow, he just focused fully on what it is that bothers her and how can he help.
