Their bodies have become quite swollen because they are full of parasites. One reason the Empire runs better than the Kingdom is that the Emperor killed some of the nobles who went against him, and took noble titles away from the ones he thought were idiots like Arche's father (she was the Worker who was killed by Shalltear in Nazarick). However, this meant he had to do a lot more work by himself, so he was tired all the time. In the Kingdom the stupid or bad nobles just do whatever they want, like kidnapping girls and selling drugs, so the Kingdom suffers.
