Ok so simple way to explain it when the movie started with the Jigsaw body, and the tape was played thats when the flash back started yes confusing at first but you'll get it someday, saw 4 is just a whole fat back flash and it was a message to the detective, that was already recruited and it shows that he was already recruited during saw 3. the detective didnt know about the tape recorder in jigsaw which is why he looked confused or concern since there might be other recruits *wink* *Wink* something like that but yea, Big flash back saw 4
