The meteor is real and it's what made everyone in the game die. The Borderland is just the place in where their mind is when they went into cardiac arrest (for only a minute). Borderland is a place where everything is possible because it's not really real, when you die in the game, it's because your body in the really world is dead. So the game doesn't take place in the hospital. They don't remember anything where they're in the real world. In season 3 it seems that like they're remembering the Borderland, but the only way to go back to the Borderland is for your real body to be in a state between life and death (coma or cardiac arrest)
