Because I'm very new to taking commissions, i have made a form to be filled out by any requester. As a practice trial run i will be only uploading the form here for the time being. Please make sure to read all the ground rules! I won't be particularly fast i don't think, and I've kept it to 3 formats that I feel comfortably able to perform.
