Sheer heart attack is a bomb created from Killer Queen's left hand and is it's secondary ability. Most stands have a main ability and can develop secondary abilities pertaining to the main one. It chases down anyone trying to escape from Kira which is probably an extension of his desire to get rid of anyone pursuing him.

2020-09-16 21:25:33 +0000 UTC*MINOR SPOILERS* He does get it to 2 seconds at the end but even then it's very little. So many reactors are always confused about why he doesn't just stop time for forever like DIO and beat everyone instantly.
