From what I can tell, the people who live in the walls pretty much became complacent with their lot in life as long as they were safe from the titans. Almost nobody is going to volunteer to actually combat titans because the world outside is chockful of them, in other words from their pov it'd be a never ending battle where you're almost certain to end up lunch. Compared to that they were fine living in the walls. Problem is that without careful management there isn't enough meat to go around due to limited farming. As it was each of the cities behind the walls pretty much only had enough to sustain themselves, so when a good population of just one city come flooding into another then yeah, ya got the makings of a famine. It's to the point where if ya want to eat meat, ya gotta leave the walls go into the wilderness and hunt it while avoiding titans.
