The girl was the boy's childhood friend that had always had a crush on him, but because she wasn't as pretty as the girl that moved away it was never reciprocated. In high school, she saved up enough money working at a bowling alley to get cosmetic surgery to make herself "as pretty" as the girl that moved away. The boy ended up frequenting the bowling alley after the surgery and didn't recognize her because of the surgery. The day he found out about the surgery and confronted her in the bus, it was to reconnect and maybe actually ask her out. So when they were brought to Quindecim and lost their memories, the girl saw a pretty face in the mirror and since she connected beauty to the girl that moved away, she thought that she was that girl. In the end when they remember, neither of them had hard feelings and they decide to spend their final moments together on the date that they never got. Because neither of them showed an ugly side, both are sent to heaven. The game only exists to put the two candidates against each other, winning and losing doesn't actually matter. The whole thing is a setup to try to force the candidates to do something bad. If one does buys into the lie enough to show an ugly side where they try to sabotage or manipulate or otherwise hinder the other, they are sent to hell. But, on the rare occasion that they don't do anything bad, both can go to heaven.
