So this is a Rule 11 question, right? The attorney filed the lawsuit based on the facts as they knew them thus far (i.e. on information and belief). It's unreasonable to expect attorneys of knowing about who an organization has or has not hired prior to discovery. Not C or D.
Sanctions are for punishing the bad behavior of lawyers, so A is super wrong.
