Not sure if this is accurate to real life but he was saying that it was popular during this time period to sever your own finger to curse someone else, essentially cutting ties with them. The other smaller finger was Maomao's, they don't really show it well enough to notice but the tip of her pinky is supposed to be missing. There's a line of dialogue when she puts on the nail polish where she says "it's better than it was before" or something like that. The timeline of it though is that they had sex and didn't know he got her pregnant. Then he went away for 3 years but the time period of his visits was every 3 months, so she had no way of knowing he was gone and couldn't write to him abroad because his departure was forced and sudden. She assumed he abandoned her, and so she resented him and had no love for Maomao by association when she was born. She caught diseases, the finger thing happened after that, and she slowly lost her mind Also the POV of the pieces was weird (understandably), but some of the people we've known in the show make cameos here, you probably noticed tbh.
