Its a random generator so as much as I wish there was a pattern to share with you its random (and unfortunate in this case) luck. I'm glad you've played so many games and not come across any bugs of note.

Played nearly 100 games of this and still haven't unlocked the 3rd stage for any of the transformations on the player avatar. Am I missing a pattern or something?

I was going to say that with that mode, when you do Hourglass+Titan+Clothing, you misplaced "Curvaceous" as "Curvacious". Put then, I find out that's actually spelled like that in British English, when referring to someone that's "Pleasingly Curved". Never mind on that one.
