The Titan Sword encases a ghastly visage of a skeletal, muscle-bound titan, perpetually bound and howling in anguish within its shimmering silver blade. A silver handle adorned with exquisitely carved details and three chain links attached where the blade starts evoke a semblance of once being tightly chained.
You gain a +3 bonus to attack and damage rolls made with this magic weapon.

Optional Evolution Stages for the Titan Sword.
Sentient. The Titan Sword is a sentient chaotic neutral weapon with an Intelligence of 16, a Wisdom of 12, and a Charisma of 18. It has hearing and darkvision out to a range of 120 feet. The weapon can communicate by transmitting emotion to the creature carrying or wielding it and can speak, read, and understand Common.
Personality. The Titan Sword's sentience is the bound titan itself, a once-mighty being from a distant plane, now eternally imprisoned within the blade. The titan bitterly resents its captors and those who dare to wield it. Its purpose is singular: to escape its imprisonment, and it will seek to manipulate its wielder into actions or plots that it believes may bring about its release.
Curse of the Titan. Whenever you finish a long rest while attuned to the Titan Sword, you experience visions of apocalyptic battles and whispered promises of unstoppable power from the imprisoned titan. These dreams subtly seed the mind with a gentle inclination towards chaos and rebellion, though they don’t prevent the benefits of a long rest unless you willingly embrace the titan's whispers, choosing power over sanity.
If your alignment is not chaotic, each time you finish a long rest, you must make a Wisdom saving throw (DC 18). On a failure, you gain a Chaos Point. If you accumulate 10 Chaos Points, your alignment shifts one step towards chaotic (from lawful to neutral, from neutral to chaotic). Once your alignment becomes chaotic, you no longer accrue Chaos Points from this curse.
If you die while attuned to the Titan Sword, your spirit is not automatically absorbed. Instead, the titan within proposes a dark pact: be reborn through its chaotic power in exchange for aiding in its release. If accepted, your spirit and the titan become entwined, sharing a portion of its chaotic
essence with you and resurrecting you with a spark of its dark power. If rejected, your spirit is
free to pass into the afterlife, but the sword remembers, and your soul forever carries a spectral chain, a quiet reminder in the afterlife of the pact you denied.
Releasing the Titan. Should the whispers and promises of the titan entice a wielder enough, they may seek to free the tortured spirit from its ethereal prison within the Titan Sword. Releasing the titan is a cataclysmic event, wrought with devastation and chaos. To do so requires the completion of a dark and ancient ritual known only to the most learned or corrupt scholars, involving several artifacts of immense power and a location where the veil between realms is at its thinnest.
The Imprisoned Titan (CR 26). When released, the titan emerges as a behemoth of pure, chaotic might, its form barely containing the violent energies swirling within it. The landscape itself seems to rebel in its presence, with the ground quaking and the skies darkening, signaling an imminent, catastrophic eruption of anarchic power.
As for its specific abilities and features, the exact nature of the titan, its capabilities, and strategies should be crafted by the GM, to best fit their campaign and challenge their players.
