This is because of chapters like that that monster is so special. Or any Urasawa's mangas in general. Can't wait for next episodes and for you to react to Pluto soonish haha

Monster of course has the overarching plot that each episode gradually builds towards, but it also has many episodes like this where within one span of 20 minutes, you get attached to brand new characters that you feel like you’ve known forever. IMO, episodes like this is where the brilliance of this series truly shines forth.

To the point that he, hearing the sounds of the candy bags, knew which candies they were likely to pull out the next time they visited. One thought I've had. Because once you pointed it out, it started bothering me too. Him getting the chemicals and placing it in (or on) the candies didn't bother me much. More so how he could have guessed those specific people would take the candy. I don't remember if the show ever provides an answer. Another thought is that he was targeting any doctor/nurse scum enough to steal candy from a kid patient in coma. And then he just happened to get lucky with it being the very doctors that were bothering Tenma.
