The damage is already done so the drama of how the 'ultimatum' Dean gave is meant to parallel John’s doesn't work here. It's like that along with the 'monster' reference had to be shoved into this overstuffed episode instead of giving time to real Sam and Dean vs their hallucinations. I wish they had given this more time to breath because the flip flopping is all over the place even for Bobby in this one.

I definitely do think that it would have been better to talk to Sam before just surprising him with locking him into the bunker - but, I doubt it would have done any good. Sam was determined that he was the only one who could take on Lilith and he was very confident in what he was doing was the right thing do to. That's my opinion. They were stuck between a rock and a hard place really. Sam was not going to willingly give up drinking demon blood and he'd just get angry at them trying to convince him to do it. I think it would have ended up in a fight anyway. Dean saying "monster" really was a trigger. He didn't know that though.
I don't think any of them saw a way that they could just stop what they were doing since they knew the seals were getting broken faster and faster. Lilith had to be stopped. Some how. But Dean was determined that it not be done through sacrificing Sam. That's how he saw it, even if Sam wanted to do it.
I truly think that Sam's thinking was strongly influenced by the demon blood. He wanted to drink it, but it also affected him and how he acted. We've been seeing it throughout the season. And with Ruby encouraging him, it's no surprise that he'd be acting like this. I doubt Sam would have acted like this before he started drinking the demon blood. Once he did, it had it's hooks in him. He's really not capable of thinking objectively anymore. And after all this time, how much has the demon blood changed him? Not just his mind? He even told Chuck he could feel it changing him.
I also think Sam has to hold on to the belief that he's the only one who can stop things. That justifies what he's doing. He's justifying what he does so he doesn't have to acknowledge that he really Wants it.
That hotel room scene was intense. The camera work really set it off too. Both of the were triggering each other, but Dean did try. He did. Sam has to work on his Little Brother issues. I understand those, since I'm the youngest too. I think Ruby did really manipulate Sam's thoughts regarding himself and Dean. She really pulled out every bit of any issues they had and helped them to grow in Sam's mind.
This sort of thing was what Dean has been afraid of ever since he became aware of Sam's powers. Then Sam starts working with demons. They learn about the demon blood Sam got when he was a baby. This is the sort of thing that haunted Dean about his little brother. And Sam knew it. Sam had fears about Dean thinking about him that way. And here it all came out.
I can see how Sam would have started working with Ruby and drinking her blood with totally good intentions. Thinking he had no other recourse. But from the start it was all flawed. And Ruby was playing him. Got him at a weak point. And then I believe the demon blood, with Ruby's coaxing, messed with his thoughts. And Dean isn't good on the best of days which emotional stuff. He's hurting too. His brother keeps choosing Ruby over him.
