I didn't get it either that Maggie was a ghost when I saw it for the first time either. I don't think many people do. I did find it weird that the girls were always in the same outfit. I think the time span was 2 or 3 days. Weird that Sam and Dean didn't stay to make sure to deal with the ghosts by burning the bodies. I think this is one of those episodes that is okay to watch the first time, but gets passed up during rewatches by some people.

That Dean would turn into the things he hunts if he refused to leave and stayed behind. He'd be stuck as a spirit in the hospital and he would change over time. Being a fan of this show can sometimes feel like going after a college degree, what with all the details of lore and such you need to remember. But that's one reason a lot of us re-watch the show over and over again. I still pick up things I missed or didn't process right on previous watches.

They said "no way" and build it themselves, for just $5,000. After the shooting, the dollhouse was donated to an orphanage. 2) About Maggie being evil: in SPN universe, it's implied that being a ghost is unnatural to a human soul after death. Being a ghost can make even the sweetest soul become angry and violent over time. That's why Maggie killed without remorse, but she still cared about her sister.
