First of all, yes Lobotomy Corp takes place in an underground facility so it being a bunker is pretty accurate. Second, each plays when the monsters you manage "abnormalities" escape. Each warning is essentially a tier of how absolutely fucked you are. First is an inconvenience, Second is a clusterfuck that can be fixed but won't be easy, Third is despair as there's almost no way of fixing the mess you got yourself into and you might as well give up. So you're pretty on point for 3rd warning being "you're too late."
Also lore wise there are 7 trumpets, 1-3 we hear the rest not in the game for lore reasons. 4th is this branch is completely lost and beyond saving. 5th is multiple branches are fucked and abnormalities have escaped into the city. 6th means Lobotomy Corp is no longer a Wing (one of the mega corps that run the city). 7th means the end of the world. They're based on the 7 trumpets of the apocalypse, as Lobotomy Corp has HEAVY gnostic theming.
