It's funny how this episode contrasts with the US Navy -- in both it and Starfleet people can be in different chains of command with the same rank, but the US Navy (possibly the same with the Canadian and UK navies as well) would not have Troi in command of the bridge because she's in the medical chain of command. The actual senior officer would have been either Ro or the other Ensign. You'll notice that on the original series McCoy was never in command of the bridge. And Roddenberry actually said that Uhura was supposed to be fourth in Command after Kirk, Spock and Sulu but the network overrode him and forced the chain of command to have Scotty and some rando named LaSalle be ahead of her. (He then went through a divorce and had a troubling misogynistic phase that at least abated before TNG, possibly before the first film, but that's another story.)
