A cat's urethral tract is very small (especially in males), so when there's an obstruction, cats can pee blood. But the amount is very little. Then a full blockage can occur where they can't pee at all. Which causes the bladder to swell up and eventually can cause death. There is a surgery that can help widen the tract a bit in male cats (PU surgery) for urination to help prevent recurrent blockages.
