Nope, that is a redhearing in reality. While Medusa has similarities with Ophis the later never split apart or lost power, remaining a single entity throughout its whole history. Medusa came from a legit goddess who made a deal and I am just not saying which one because spoilers.

As a family the Emiyas are a loving one and so Shirou having a Reality Marble or not doesn't affect their dynamic. Sure Kiritsugu will have more to say, paranoid as he is, but only Illya and Sella would activaly worry about it because, well, Khaos Brigade.
