The idea that a demon can be summoned to stop outbreaks of madness ( in the most evil way, by killing the victims of mental illness), is really some out-of-the-box writing. However if you look at history of evil regimes, that is what those regimes resorted to, so the idea of it is not so far fetched in the Buffy world. Still -- horrific when you think about it. The monster was gross enough, thank goodness we did not see it kill more than once. The editing of the action scenes and the lighting especially in the Summers house was like a top tier horror movie.
One thing I noticed is that Buffy is really off her game, because of Joyce and how to deal with that and with Dawn. She was stopped by Spike and totally distracted from the monster dealing with him just walking in the middle of the night. Not even really mad, like just mildly annoyed, but she forgot about the monster in the house, not like her.
And then the twist at the end -- wow way to hook you into the plot so much deeper.
Spike really needs his ass kicked, he is messing with the slayer in a sexual way and that is going to be ugly. Riley is in too deep and Buffy will never understand cheating on her that way. All these dirty secrets have a way of coming out and blowing up eventually, and Buffy is always right at the heart of it. Its all very scummy and Buffy is ready to explode on someone, it feels like.

Small thing I recalled for the mental illness candidates seeing Dawn as not belonging... I'm not sure where I picked up this from exactly however, there were beliefs floating around that people considered "crazy" by some cultures were said to have visions or contact with another world. Possibly some kinds of prophets, shamans, medicine men, or witch doctors could fall into this category.
TLDR: The mental patients are seeing Dawn as she actually is due to seeing into two separate worlds at the same time.

Also she wasn't consistently mentally unstable. She was just having bouts of issues that came and went. Was probably confusing to the creature.

Maybe its just the fact that the monster is from outer space, but the monster design of the Quellor demon always felt more 'X-Files' than 'Buffy' to me.
Its cool that Riley got a chance to shine intellectually with these soldiers. When he was introduced he was the smart psychology guy, the TA to the (evil) genius professor; but later in S4 they started acting as if Riley was "just a soldier", just there to follow orders. Even in this episode he kind of references that with his 'I'm not so great with the research' comment but then he figures out chemical compositions of the goo, figures out the best way to track it via radiation and figures out where the monster is going when the soldiers were going to take their time with going through all patients released.

Hey Cassie. It looks like the whole mental patient explosion isn't quite connecting for you. All these people have been brain sucked by Glory. Ben was trying to get rid of them because so many are showing up. And yes the Queller may have been assessing Joyce because her mental condition isn't caused by Glory but it could still detect the mental problem so it was hesitant.

actually, Graham didn't talk shit about Buffy ever; last time he just told Riley that he needed to do something beyond being the "mission's boyfriend" which.... he was right. 👀
Forrest was the one talking shit about Buffy all the time, while Graham was out there complimenting her on her skills.
