I don't own Resident Evil 4 so I can't mod it properly but I saw this jeans mod and it felt like it needed a diaper underneath.
Diapered Ashley Mod and a Playable Ashley Mod addon that replaces Leon. You must have the Diapered Ashley Mod on first to play as her.
Update: The Ashley mod works but the playable version doesn't, Unfortunately I'm to tired right now to figure out what I did wrong with it but I will look into in the future.

This mod adds a pacifier to every Claire costume and mod, I have only quickly tested it, so if you find any issues please let me know.
I recommend adding this mod last to any other mod, this mod replaces hair mesh by adding the pacifier, any mods with different Claire hair won't work.

I still have so much to learn with this game but I have now managed to get the weights working to make it look like she has a diaper under her pants.
Unfortunately, the skin that I used isn't unlocked till further on into the game. I will try and make something closer to the beginning of the game in the future for those that haven't played it yet. You can download a complete saved file and get it that way and replay missions.
