Werebats have existed before in D&D. This is my particular take on the concept.
Werebats have a mystical tie to vampires. They hunger for blood, and drinking it can quickly heal them, but they don't need it to survive. Vampires can control werebats...which may lead to werebats that have an intense hatred for vampires.

As part of my project to add new lycanthropes/werecreatures for 5e, I created the werecrocodile.
These ambushers are largely antisocial, but find acceptance (and willing followers) with lizardfolk (as long as they don't turn back to mammalian humanoid form).

A creature with "giant claws" (the name combines the Slovak words obor and pazúr), this monstrosity is a Huge replacement for the D&D umber hulk. Bigger, with more dangerous melee attacks, this creature's scent causes delusional thoughts in creatures near it (rather than relying on a PC looking at it).
An oborpazúr might be enslaved by a jangboko, which makes both creatures more dangerous to adventurers...

I've modified the 5e lycanthropes somewhat, mostly changing things like their damage immunity (now a resistance, but no longer bypassed by magic weapons) and giving them a little regeneration (which gets faster when there's a full moon).
This includes the werebear, wereboar, wererat, weretiger, and (of course) werewolf.
Not major changes, but it does increase the importance of silvered weapons (so they're not something to just throw away as soon as you get a magic weapon) and make...

Jon's cat, Sir Fluffles, has gone down into the basement and hasn't returned. Jon is afraid of the dusty old basement, so he begs passing strangers to rescue his kitty. He'll even give them gold as a reward.
It's just an ordinary, dusty basement. Probably the easiest rescue quest ever. I'm sure there's nothing out of the ordinary down there.

The players in my campaigns have encountered this, as it has been a part of my homebrew world for a very long time.

A part of D&D for, like, ever, the nymph did not return for 5e. I have need of one for an upcoming unique foe so I decided to stat them out based on an older edition of the game.
They don't have high AC, or a lot of hit points, or a lot of combat abilities...but their supernatural beauty can blind or even kill you.

This legendary magic weapon is not only a tool of the battlefield, it grants you the power to (possibly) foretell the future.
The sword does let you use a 9th-level spell, but at the cost of it losing all of its magic for an entire month.
And yes, it does have what looks like a small version of Magic 8-Ball for a pommel. Instead of a floating polyhedron, this item uses actual magic.

In a world where friends, charm person, suggestion, and dominate person are a thing, shopkeepers need to protect themselves from people (aka, adventurers) who would use such magic to cheat them out of hard-earned money.

The slimy red-orange goo inside this wooden pail won't spill out, but you can pull a handful out and throw it at something. When thrown, it becomes an improvised ranged weapon that deals acid damage, and it deals extra damage to metal things as they instantly corrode.

This massive, ancient tortoise likely doesn't care a bit about a group of adventurers, not even noticing if they are underfoot.
The PCs in my current campaign encountered one of these. Where it had been sleeping, they found the remains of a crushed house, and in the basement space were the remains of another group of adventurers that never got out when the tortoise decided to go to sleep years earlier.

Be able to see even when you normally wouldn't be able to. Only works for the caster, doesn't let you see as far as darkvision does, but reveals things more clearly and gets around things like fog.

Conjure a dagger or shortsword made of fire. It works a lot like a normal weapon, except it does fire damage, uses your spellcasting stat for attack rolls, and it reappears in your hand if you throw it (dagger) or drop it (dagger or shortsword).
When you go up in level, you can make two of them appear at once so you can dual wield if you want to (you could cast it twice to do the same thing at lower levels).
