After you signed up on discord, please use a web-browser (NOT the phone) to go to this link to connect your Patreon profile to the Discord profile and get all the access: https://www.patreon.com/settings/apps/discord
However, if that doesn't work for some reason, there is another way to connect the accounts, just go directly to https://www.patreon.com
Then sign in with your credentials.
Then go to "Settings"
Then under "More" you will find "Connected Apps" - click on it.
Then click on - connect discord to Patreon
