Toa got pushed too hard, struck back, and Jin died, likely from her skull hitting a table edge. Then Toa got pushed into taking her place. When it looked like Toa was going to get married and leave all the ladies in waiting to deal with the fallout, they panicked and used Jin’s favorite poison to kill her and call it suicide.

Whoever is behind it doesn't actually know who's pregnant which is why they were selling those garments last episode to see who would buy ONLY garments for pregnant women. Maomao clocked that and said hmmm let's get a variety so it doesn't look suspicious.
