They just don't have the ability to fight off in their stomachs botulism which can form in honey. Again the show echos the words stated in episode 1 that ignorance can kill.

The line is saying that when Ah Duo saw during childbirth that the empress dowager and her son took priority over her and her child, she decided to swap their babies in order for her son to have a better life.
