It's a little unclear here, but the way Subaru figures out Echidna knows about RbD is because she isn't surprised at his immediate return, as if she was expecting it. From her perspective, he should have just left the first trial, only to suddenly lose his mind upon waking up and beg "WHY". So when she pulls him in and doesn't comment on how he lost his mind so fast, he notices and realizes she knows there are lost memories in between as a result of RbD.
