The handling of the blood vessels is the primary concern in such a procedure. Therefore they are performed by vascular surgeons. Ask basically any larger modern hospital which subspecialty of surgery is performing the amputations.

Jigsaw is RIGHT THERE, IN PERSON, CLEARLY ABLE TO SEE that the people he is currently testing are all actually following through with your requests and putting themselves through incredible suffering in order to continue living. You can SEE IT HAPPENING, Jigsaw. You maybe want to forego the time limit for some of them for actually doing what you asked? You maybe want to reward them for “rediscovering their survival instinct”? Or is your criteria for passing the test seriously going to boil down to a time limit? Come on man.
