UPDATED WIGGLE REWARD FORM!

For organizations sake on my end, I’ve added a spot for people to add their name here on Patreon as well as their email, so I can make sure I have the accurate forms I need for each month :”3 because I’ve cleaned it out I will also need entries again from people SO!!
