this is a show with soooooooo many different characters and it's super hard to follow the story, even the native English speakers have problems following :) so don't worry!
i will try to give you just a little bit of information ^^ first, about animals:
Stark - wolf
Lannister - lion
Baratheon - stag
Targaryen - dragon
those are most important for now. basically every house has an animal that represents it and it's usually in their sigil/flag.
secondly, the sisters from this episode: originally, they were Catelyn Tully and Lysa Tully (speaking of animals, the Tully animal is fish btw!), now they are - Catelyn Stark and Lysa Arryn. Lysa's husband was Jon Arryn, the Hand of the King for 17 years, who was murdered, and Cat's husband is Ned Stark, the current Hand of the King.
and yes, Lysa's son Robyn is too old to be breastfeeding, that's not normal anywhere, even in this world.
lastly, you seemed to be confused about the woman Cersei was asking her husband, King Robert, about - that's Lyanna Stark, Ned Stark's sister. maybe you remember from previous episodes when King Robert was talking about her, the only woman he ever truly loved. you will hear the term "Robert's rebellion" a LOT, that's the war against the Targaryens that Robert started, because Rhaegar Targaryen kidnapped and murdered Lyanna Stark. THAT's when Baratheons & Lannisters defeated Targaryens, Jaime Lannister stabbed the Mad King (Aerys Targaryen) in the back, Robert Baratheon became the new king and married Cersei Lannister (and now they have three children).
this is probably also a lot of information. but let everything sink in, perhaps even rewatch the episodes or parts of them... when I was watching the show for the first time, I also missed a LOT of things, and re-reading and re-watching helped me a lot :)
all of this is spoiler-free because it was all mentioned and explained in the show already! good luck and enjoy the show! i know it is very brutal with a lot of explicit, gory and sex scenes, which you are probably not really used to :blush:

When they said the Wolves and the Lions will be at each others throats they’re talking about the Starks and Lannisters. Basically anytime they talk about lions and wolves that’s their house sigils. They speak in a lot of metaphors back then cause the language is more young. Not as many words in the dictionary back then

Don't feel bad about having difficulty following some of the conversations. Especially with Little Finger (Lord Baelish) and The Spider (Varys). These two always use SUBTEXT when they speak, and you have to figure out what they really mean. When Little Finger and The Spider meet in the throne room they have what seems like a friendly conversation on the surface, but they are threatening each other, and assuring the other if you tell my secrets to destroy me, I will do the same to you.
Ned Stark is easier to understand because he always just says what he's actually thinking. An honest man is at a disadvantage in this place.
The Breastfeeding Scene. This is supposed to be shocking and establish that Lady Stark's sister has gone mad, and THE LORD OF THE VALE, Robyn Arryn, is young but is mentally even younger. His mother has not allowed him to move past the infant stage. He's like a baby but has a lot of power. Yeah, he's like KING-BABY :) . Lady Stark went looking for help from her sister, but just finds herself in another dangerous situation.
Hella asked a really interesting question during this reaction. "What if kids saw this?"
Game of Thrones originally aired on HBO which is a channel you have to pay extra for, and their content is not meant for children. It's like the opposite of the Disney Channel. But, I'm sure accidents happen and kids will watch something before they are supposed to.
As a kid, i would wake up late a night to watch horror movies my parents thought I was too young to watch. I would get so terrified I would stay up all night, and at one point I looked so tired, my mom took me to the doctor because she was worried I had cancer.
In the US, we have have the term KINDER-TRAUMA (This is a play on the word KINDERGARTEN which is where we go to school as very young children). But, Kinder-trauma means you watched something too early as a kid and got traumatized or you watched something meant for kids but it contained something that scared you so bad you never got over it.
