Often times repeated stretching exercises with some ointment (steroid cream typically) does the trick. Maybe get a second opinion before you do any surgery as circumcision is still done for cosmetic/religious reasons and thus may be recommended prematurely.

Imagine these people doing a good job at teaching hygiene in general. Made me thinking of the story qt told us about how she learned about tampons and how secretive and how much shame surrounds the topic in the US. It's got to add to the problem. Sex-ed is more important even for other life-skills than many people think.

But in that case sex would be painful anyway so he'd want to see a surgeon over a sex worker. I know Sex Ed. is really poor in some places but I just can't imagine being that unclean and not doing something about it (I know depression can affect hygiene but even then that's no excuse not to clean yourself before sex).
