The crew members mess up or get into mischief sometimes but when they REALLY screw up or annoy the Commander, Commander Shepard humiliates them in various ways to punish them.
Here's what the crew did to deserve punishment:

EDI: Teased Joker too much, again.. But this time it was to the point where he almost crashed the Normandy.

Miranda: Went against Shepard's orders, when the Commander lectured her she got angry and in her brief outburst said Shepard's dick wasn't as big as he thinks it is. Ouch.

Liara: Almost got the squad killed when she couldn't help herself and went back into the collapsing building to grab the Prothean dildo-esque artifact.

Shepard: The commander's bad decisions got [CLASSIFIED] to take heavy actions against [CLASSIFIED] resulting in the total destruction of [CLASSIFIED], The Commander chose to humiliate herself in front of the crew.

Tali: Jury-rigged the shuttle without telling the Commander, ended up crashing the shuttle in a critical mission. Suitless Tali got sick after the punishment. Commander Shepard felt bad about it afterwards..

Samara: Kept interrupting fraternizing crew members, including Commander Shepard. Walked into their private time and lectured them about discipline, purity and its effects on the mission.

Ashley: Chased after an important target without Commander Shepard's permission, ended up running into an ambush and got captured. The Commander had to fight through hundreds to rescue Ashley.

Kasumi: Stole Commander Shepard's pair of sweaty boxers and Shepard caught her smelling it in secret. Crew members also reported missing boxers/panties. Weird.. Usually Kasumi only steals valuable things...

Jack: Jack was, well... Being Jack, she's used to punishment and seems to even like the humiliation. Since the punishments got introduced she's been getting into a lot more trouble than usual... The Commander thinks we need to think of some other punishments for her.. As this one seems very ineffective.
