Jeffrey Dean Morgan had a contract at this time to appear on 'Grey's Anatomy' and in fact was travelling back & forth fron Vancouver & LA to do both shows. That's one of the main reasons they killed off John. Also, the series is really about the boys, not the boys and their dad. And nobody cares about the damn nurse. The scene was just to show how ineffective Dean in his current state was, and that something (the Reaper) was in the hospital.
