For Medievalism I recommend using a shader with POM support. My personal preference is currently SEUS HRR 3. Some textures like glass look much better in SEUS PTGI and the image is sharper but it does not support POM which greatly enhances the textures.
INSTALLATION:
Resource Pack:
Resource Pack installation is straight forward. Just drop the downloaded resource pack(s) to the Minecraft resource pack folder:
.minecraft\resourcepacks
Mod:
Mod installation requires Minecraft Forge (always download recommended).
Run the .jar installer file and install.
Open Minecraft Launcher and select the Forge launch instanceCreate "mods" folder in .minecraft folder and drop the latest Optifine file (same version as mod and Minecraft) into the mods folder (no need to install Optifine separately)

I'd also recommend allocating a bit more RAM to your game instance in the Minecraft launcher.
