2 more Treks comin at ya! Feels like there's a lot more action in the series now, we had bomb attacks, guys running rogue around the ship and Picard and Crusher being taken hostage. And I think this is the first time I was anxious for the opening theme to be over so we could get back to the action. Engage! 🖖

I'm sure it's been addressed by now but just in case: you can't just beam on to someone's ship if their shields are up, using a typical transporter. Even the Enterprise transporters cannot beam through their own shields (hence why in The Defector they couldn't just beam Jarok off his scout ship while the warbird was present). This "dimensional shift" wasn't a traditional transporter. It was really just a technobabble macguffin to make this small group of terrorists a significant, believable threat by allowing them to pop in and out anywhere, even on the Enterprise. But they paid a price with the way it damaged their bodies. It was used very, very effectively in this episode and really helped ramp up the tension. In the previous episode, Danar's enhancements enabled him to manipulate and get the better of the crew. Q, of course, operates on a level far beyond anyone's understanding and his disappearing really isn't beaming either. But he's not bound by the laws spacetime so he can appear wherever he wants, whenever he wants. It's curious that they chose to place two such episodes back-to-back. The writers of TNG were good at finding ways to exploit the Enterprise's weaknesses anytime they needed to get someone on board, and while it's not a regular event I won't say it'll never happen again. But your understanding is correct, if the shields are up, a typical transporter can't just beam someone over to the Enterprise. (If shields are down, it's fair game.)

Crusher can give orders to Worf (or most other people) because she holds the rank of Commander, the same rank Riker holds. Riker has more positional authority as the First Officer, but pretty much everyone else (at least that we, the audience, know about) except Picard is outranked by Crusher and has to follow her orders. Even Picard has to defer to her when it comes to medical matters for the crew.
