Like yeah, his upbringing and blablabla but it doesn't even come close to what Sakura went through and she is not a shitty person like him. Also, in the VN the rape scene is way worse. I watched the playthrough of another Youtuber and he couldn't even read it because of how disturbing it is.

However if try to do it, Rider ends up killing you. She is in the room unmaterialized watching everything unfold. That's also why Sakura, before leaving the house, thanks Rider for not killing Shirou.

If it had been Kiritsugu he wouldn't have hesitate or think twice about killing Sakura. That's why when Shirou is asked by his inner self ( or consciousness if you will) "Are you going to betray?", he is not talking about betraying Sakura; rather about betrayal his own self because by choosing not to kill Sakura is the equivalent of allowing her to kill people and therefore betraying his own ideals of a being a hero, which he comes to accept by replying "I will betray"
