Remember, Tsunade's grandmother Mito lived well through the reign of the Third Hokage (meeting Kushina and all) and Kushina survived having Kurama extracted from her body. Since Part 1, Tsunade has displayed great durability, survived mortally fatal wounds such as having through her lungs, stabbed multiple times, etc..

Her lower body was detached from her upper body and yet she is alive. She doesn't have that 100 healings jutsu working either bcuz she already used it. So by all logic you should be dead, but she is still alive. Lol That's anime logic for you 🤔🤔🤔
