Buggy's big red nose is in fact his real nose. I don't think they ever discuss it, but it is confirmed to be. I guess he has no choice about being a clown pirate, everyone would call him it anyway. If you've read all of Volume 23... then math would suggest you'll see One Piece episodes where unfamiliar things happen like a bit after 2 years of One Piece reactions at 2 a week. I still look forward to every episode though.
