Just a note here about when and why officers in Starfleet are addressed as “Mister.” —
Traditionally in the British army, anyone of the enlisted ranks of rank of Sergeant Major and below and anyone of the commissioned officer ranks of Captain and above are addressed by their rank—Captain Apollo, Colonel Tigh, Chief (Petty Officer) Tyrol, Sergeant Jones, Corporal Smith, etc.
However, if you were above a non-commissioned officer rank (basically, corporals and sergeants), but below a captain—essentially, lieutenants, sub-lieutenants, ensigns, and warrant officers—then you would be addressed with “Mister.”
Roddenberry picked up this usage for junior officers (lieutenants and ensigns). So that’s why it’s Mister Sulu and Mister Chekov. (Mister Spock is kind of a special case. He should be addressed as Commander Spock, because he is a senior officer, but whatever.)
During the long era of fanfiction and authorized and unauthorized short stories and novels between 1969 and 1979, it became standard for “Mister” to be used as a non-gendered form of address. And this was picked up by the official usage.
That’s why you hear junior officers who are women addressed as, for example, Mister Saavik.
But anyone with a higher rank or title gets addressed by the higher title.
Doctor McCoy, Nurse Chapel, Doctor Crusher, and Counselor Troi are special cases because they have professional titles that trump their rank. It’s neither wrong nor a faux pas to say (Lieutenant-) Commander McCoy or (Lieutenant-) Commander Crusher, but out of professional courtesy, they’re usually addressed as Doctor instead. Same with Lieutenant Commander Deanna Troi.
When addressing a Lieutenant-Commander, it’s common to drop the “Lieutenant” part and just say “Commander.” Similarly, Worf starts out as Lieutenant, Junior Grade, Worf, but that’s commonly shortened to Lieutenant Worf. Similarly, Chief O’Brien is actually Chief Petty Officer (C.P.O.) Miles O’Brien. But informally, “Chief” is enough.
Star Trek doesn’t usually mention ranks below chief petty officer, and just resorts to “Crewman” for lower non-officer ranks.
