Fives died way before Echo was saved, Echo was saved by Rex and Anakin. Also, Tom Kane’s final performance before he had to retire after suffering a stroke.

That he had powerful visions of the future and wanted to make an army. So the Jedi just assumed that when they found an army that was signed for by him that he had just gone behind their backs. The Jedi found out that Dooku had him murdered by the Pykes in Season 6 of the Clone Wars, but they were already trapped by the Clone Wars so they couldn't do anything about it. The Kaminoans thought Order 66 was a defensive measure against Pong Krell types going on a rampage, or trying to force the clones into overthrowing the government.
